Relationship between Age of Smoking Initiation and Obesity in Korean Adult Men

Abstract
Background: Previous studies have suggested that smoking has a dose-dependent positive association with obesity and abdominal obesity. Moreover it is reported that early smoking initiation increases the risk of smoking-related disease. We studied the relationship between age of smoking initiation and obesity, abdominal obesity in Korean adult men.

Methods: Total of 4,386 men aged more than 20 years old from the 4th and 5th (2007-2010) Korean National Health and Nutrition Examination Survey were included in this study. We excluded past smokers. Subjects were classified into four groups by age of smoking initiation (nonsmoker, Group I: Age on starting smoking ¡Ã20 years, Group II: 18¡Â, £¼20, Group III: £¼18). Obesity was defined as body max index ¡Ã25 kg/m2 and abdominal obesity was defined as waist circumference ¡Ã90 cm. The risk of obesity and abdominal obesity was analyzed by multiple logistic regression analysis after adjusting confounders including pack years.

Results: Age of smoking initiation was significantly associated with the abdominal obesity. After adjusting for confounders including pack years, odds ratio (95% confidence interval) for abdominal obesity compared with nonsmoker was 1.08 (0.78-1.50) in group I, 1.40 (0.95-2.03) in group II and 1.58 (1.07-2.33) in group III, respectively. After restricting the analyses to current smoker, we found significant association of age of smoking initiation with abdominal obesity (P for trend=0.03). But there was no statistically significant relationship between obesity and age of smoking initiation.

Conclusion: Adjusting life time amount of smoking, early smoking initiation is positively associated with abdominal obesity in Korean adult men.
